Check out ebsuite.com, We have been using it at Allied for about 6 months. We use Remedy before, very expensive$$. The application has everything for customer support, the web portal even lets us configure a page for each customer. So our FedEx page contains their logo and the DHL page contains theirs.

EBSuite has the most robust configuration for case escalation and alerts everywhere, on-screen, email, pager. There automatic case creation response templates keep our us and our customer notified for at each case update.
Automatic knowledgebase create from case history is great and fast.
At $25 per month/user it is by far the best value on the net for hosted services.
